Referrals:

Please make sure you have a valid referral from your GP or Specialist. If you are attending as a couple, please ensure both you and your partner are listed on the referral. This is a Medicare requirement. GP referrals are valid for 12 months whilst Specialist referrals are valid for 3 months.

If you are referred by a Specialist you will be required to obtain another referral from your GP before your Specialist referral expires.

Appointments – general:
Please bring your GP/Specialist referral with you to your appointment. Please make sure you arrive 10 minutes prior to your appointment time so we can ensure we have all the information needed before you see your Specialist.

Appointments – donor programs:

If you are a patient of Fertility SA, you will need to make an appointment with your specialist to discuss your suitability to become a recipient of donor oocytes/gametes. If you feel that this has already been discussed at previous appointments please advise Fertility SA and we will confer with your specialist to confirm whether a further appointment is required.

If you are not a patient of Fertility SA, you will need to make an appointment with one of our specialists to discuss your suitability to become a recipient of donor oocytes/gametes. You will require a valid referral from your GP or Specialist for yourself and your partner and will be charged our initial consult rates for this appointment. Payment will be required in full on the day of consultation.

If your specialist feels that you are suitable to become a recipient of donor oocytes/gametes, your initial appointment fee will be deducted from your Preliminary Work up Fee.

Patient fees:
All accounts must be paid in full on the day of consultation. We submit all accounts to Medicare using on-line claiming. Please advise us if you would prefer to take your claim to a Medicare Office instead. Private health insurance does not cover consultation costs unless you are an inpatient in hospital.

Interpreters:
Please note that any costs for interpreting services will be invoiced to you. You will receive this invoice after your appointment. If you do not attend your appointment and do not advise us that you are unable to attend, you will still incur a charge.
